Join the NSW Regional Cancer Research Network

Use the form below to join the NSW Regional Cancer Research Network. Once submitted, your request will be received via our Jotform platform.

Before you submit

  • Please complete all required fields before submitting the form.
  • At the end of the form, you will be asked to consent to your email address being shared with the Cancer Institute NSW to enable full access to the Cancer Research Education Portal (CREd) and related events and resources, including the NSW Cancer Research Seminar Series.
  • Access to the CREd Portal is managed by the Cancer Institute NSW and is provided by email invitation only. To receive an invitation and gain access, you must select “Yes please” when prompted.
